TNF- Alpha is a proinflammatory cytokine with an ability to kill tumor cells and induce hemorrhagic necrosis.

Mainly macrophages and monocytes produce TNF but activated T cells, mast cells and keratinocytes may also produce it to a lesser extent.

TNF-Alpha has a number of effects including apoptosis, adhesion and cellular trafficking, angiogenesis, myocyte proliferation, fibrosis, phagocytosis, cytokine production, leucocyte/macrophage function, inflammation and tumor control.

Interpretation

The determination of autoantibodies against tissue transglutaminase helps in the serological diagnosis of gluten sensitive enteropathy such as celiac disease and Duhring's dermatitis herpetiformis.
Ttg test is a screening test to identify celiac disease.
Additional testing with Endomysium antibody and Gliadin deamindated peptide (GAF-3x) may improve the diagnosis of disease.

73     TRUE CUT BIOPSY 500
74     TRYPTASE - SERUM 6000   <= 11.00 g/L

1. Tryptase is the most abundant protein in mast cells. During IgE mediated allergic reactions mast cells are activated and release inflammatory mediators including tryptase.

2.ImmunoCAP Tryptase measures the total tryptase levels including all forms of α-tryptase and -tryptase.

3. The baseline level of tryptase in the circulation reflects the number of mast cells.Elevated basal levels of serum tryptase and/or an underlying mastocytosis may be risk factors particularly in patients with history of severe reactions.

4.In anaphylaxis the triggering agent should be identified.Allergens commonly implicated in anaphylaxis are insect venoms, drugs, latex and foods.

5.Specific IgE antibody testing aids in the diagnosis of the triggering cause of anaphylaxis.
